What information does Dr Pauline Tapping collect?

We will collect medial details (named GP and medical practice). We may also collect more sensitive information such as current and historic mental health difficulties, medication used. On occasions we may also ask for school / college details or the details of other professionals who are involved in the care of the client.
As the 'data controller', Dr Pauline Tapping is responsible for ensuring that the processing of the data complies with the data protection law.
Dr Pauline Tapping will also process data in accordance with legitimate interests associated with the business for example: generating invoices and receipts, and accounts / tax returns.
What does Dr Pauline Tapping use the collected information for?
The information will be used to provide psychological services, as part of a wide range of psychological models and coaching theories. Dr Pauline Tapping will also use your data to contact you should there be any changes to the agreed appointment time or location, or in the event of unforeseen circumstances.
On occasions the information may be used in order to carry out administrative, ethical, legal or contractual requirements.
Dr Pauline Tapping will seek permission before contacting the client to provide information on additional hosted activities, such as training, workshops or email mailing lists / newsletters.
Information on how Dr Pauline Tapping processes and stores your information
Dr Pauline Tapping is committed to ensuring your privacy is protected and that identifying information is held safely and securely. Paper documentation and correspondence is kept in a locked filing cabinet. There may be occasions where paper documentation is scanned and in these cases it will be kept on a secure password protected device which is protected by up-to-date virus software. Should any report be requested for the client this will be password protected for additional security.
Dr Pauline Tapping will take every reasonable measure to ensure the safety and security of personal data against viruses or malware. However, Dr Tapping cannot be assume responsibility for all communications being virus free.
In the event of a data breach, the ICO will be informed within 72 hours.
Dr Pauline Tapping will respond to personal requests for data held about them within one month (30 days). Any requests must be clearly reported as a data subject assess request.
What information does Dr Pauline Tapping share?
We are committed to protecting information held about you. There may be occasions where we may need to share information about you with other professionals / organisations
-Consent to share - you may consent to the sharing of information about you to named professionals or others involved in your care or wellbeing.
-Clinical Supervision - within British Psychological Society Guidelines (BPS), Psychological therapists are required to receive regular supervision. A named supervisor will be used who is accredited by the BPS and governed by the HPC. Any clients discussed in supervision will be referred to by initials only or a pseudonym.
-If there is a serious risk of harm to self or others.
-Legal duties - for example if Dr Tapping is legally required to share information relating to terrorism, safeguarding or criminal matters.
What are your rights?
Your rights include the following:
-To be advised about how Dr Pauline Tapping uses your personal data.
-To request a copy of the personal information that is held about you.
-To amend any errors or inaccuracies contained in the data that is held about you.
-To request that your personal data is removed or 'forgotten'. This 'right to erasure' or 'right to be forgotten' is subject to some restrictions and is only applicable in certain circumstances. How long does Dr Pauline Tapping keep information held about you for.
Dr Tapping does not keep your data for longer than is necessary, and we collect the minimal amount of personal data that is required in order to provide psychological services.
Administrative data will be kept for up to 7 years if this is necessary before being destroyed. This is inline with guidance from the BPS and HCPC. NHS guidelines state that for children under the age of 18, personal data must be retained until they are age 26.
